PARTY SQUAD RENTALS — SAFETY, RISK & CLEANUP WAIVER

I confirm I am at least 18 years old and authorized to request this rental. Bounce houses, slides, games, tents and other equipment involve inherent risks including falls, collisions, equipment movement, injury and property damage. I accept the ordinary risks of proper use.
I will follow all written and verbal safety instructions, supervise participants at all times, follow posted age, height and occupancy limits, keep shoes, food, drinks and sharp objects out of inflatables, and stop use when weather or other conditions are unsafe. I will not move or alter equipment and will report damage or safety concerns.
UNDERGROUND UTILITIES: Before delivery or setup, I am responsible for having underground utilities and other buried lines in the setup area located and marked, and for telling Party Squad Rentals about their locations and any hazards.
ANIMAL WASTE AND CLEANUP: I will keep animals away from the rentals, remove and properly dispose of animal waste from the setup area before delivery and throughout the rental, and return rented items and the area free of animal waste, trash, food, mud, grass and other debris. I will not use anything that could damage the equipment. Additional cleaning or repair charges may apply as stated in the final rental agreement.
To the extent allowed by law, I release Party Squad Rentals LLC, its owners, employees and delivery crew from claims for injury or property damage arising from ordinary rental, setup or use. This does not release claims that cannot legally be waived, including gross negligence, reckless conduct or intentional wrongdoing.
This form is a rental request, not confirmation of availability. Party Squad Rentals must confirm selected items, date, delivery/setup details and final terms before the reservation is confirmed.
